  • Description of Programs

    The LEHB Health Transformation Program is a FREE program offered to all covered LEHB members including officer’s spouses and dependents over age 18!  You are given the tools and resources needed to improve your health including a personalized exercise program and nutrition plan.  

    We also offer general guidance and the potential for monthly cash incentives for your hard work along the way!  This program runs all year long, and you can join at any time.  Once you have been in the program at least one year, you are eligible to compete for the annual grand prize of $5,000!


    The 12 Week Fitness Challenge is typically offered 1-2 times/year with specific registration date deadlines.  This program is heavily advertised for over a month prior to its start date to current participants and throughout the department.  You have access to all of the same resources as the Health Transformation Program, but you have the opportunity to set more intense, short-term goals, and have the potential to win cash prizes weekly by participating in mini-challenges along the way, and compete for a grand prize of $1,000 at the end of the 12 week period!

    YES, you can be part of BOTH programs, and we encourage you to do so!!


  • What do we need from you?

    To begin either program, you will need to register, fill out a questionnaire, and complete measurements either on site or fill out a remote form from home.  The only difference between the programs is the timeline for the 12-week challenge has specific date deadlines for each step, in order to be eligible for the grand prize.  More details of each step are offered below.  


    To remain in each program, you need to complete measurements at least 2x/year for the Health Transformation Program (ideally every 6 months) and complete a pre and post 12-week measurement for each challenge.  We may also request a questionnaire update periodically and check on you in general to see how you’re doing.  


  • What is included?

    - The opportunity to meet one-on-one with a fitness professional for your assessment

    - FREE access to a fitness app that contains a personalized exercise program and nutrition goal just for you!  This app includes all resources needed to help you become successful, including videos of trainers showing you exactly how to complete the exercises!

    - Exercise tips and personalized education sent to you throughout your wellness journey

    - An 8 Step Nutrition Guide designed especially for officers but includes great advice for all!

    - Unlimited email and phone access to your fitness professional!

    - Opportunities to have fun, lose fat and/or build muscle, and win cash prizes!


  • Prizes and Giveaways

    Mini-challenges are offered throughout each program that will ask you to complete a specific goal in a designated period of time.  You will have incentives to create small short-term changes in order to set better long-term habits for yourself, and you will be entered into a drawing for cash prizes, JUST FOR PARTICIPATING!  

    Grand prizes are awarded for those with the best overall participation and progress.  For each 12-week challenge, there is a grand prize of $1,000!  That’s right!  You can earn $1,000 in just 3 months and feel incredible too! For the Health Transformation Program, participants who have been enrolled and engaged in the program at least 1 year by the January annual award date are eligible for a grand prize of $5,000!!!


  • Step 1 - How to Register for a Program

    To register for one of our programs, you will enter some general contact information, and fill out a Health History Questionnaire.  This questionnaire is used to create your personalized program, so it’s an important step.  

    Some notes for registering:

    - The information you enter will automatically create your record, so please double check all contact information for spelling and accuracy.

    - Your email account becomes your username for your Trainerize account, so please keep your email consistent on all forms.  It also must be unique for each account, so family member registering together, please make sure to enter a different email for each of you.

    - Your ID number in our system will always be tied to the officer’s payroll number.  For the officer, it is the actual payroll number (i.e. 123456), the spouse would add an “a” at the end (i.e. 123456a), and each dependent would add a different letter after starting with “b” (i.e 123456b for Dependent 1, 123456c for Dependent 2).  Once you have created an account with your ID, please remain consistent using this for future entries.

  • Step 2 - Fill out the Health Questionnaire

    You should be prompted to fill out the questionnaire when you register.  If not, or if you were unable to complete it right away, you can still fill out by clicking below.  It only takes a few minutes, and the information is kept confidential and only used for Damien to create your personalized program.

  • Step 3 - Complete Measurements

    In-Person Assessments

    For each 12-Week Challenge, we offer many appointments at multiple sites to meet with us for the 2 weeks prior to the first day of the challenge, and for 2 weeks after the end of Week 12.  If you miss the 2 weeks of measurements prior to Day 1 of the Challenge, you may still submit measurements remotely (see below), and you will still receive all the resources offered with the program.  You will be able to participate in all mini-challenges after you join, and eligible for drawings for weekly cash prizes for participating.  You just won’t be eligible for the $1,000 grand prize at the end of the Challenge.  

    For the Health Transformation Program, we offer periodic measurement appointments on site throughout the year, which usually depends on the number of registrations each month.  We ask you get measured as quickly as possible, ideally within the first 30 days of joining.  If you have registered, and you don’t see any upcoming appointments available, please contact us.  

    When you complete your measurements on site, we ask you arrive in clothing that is as form fitting as possible.  If you are measuring at work, we will have the best accuracy if you are able to go down to a base layer of clothing (typically work pants and t-shirt under the uniform) and remove all items off your belt.  (We do have a screen on site for privacy).  You will be asked to remove your shoes to step on the scale, and pose for a front and right side profile picture (also with shoes removed).  Super quick and easy!!  You will receive your measurements by email within 7-10 days during a Fitness Challenge, and even faster with the Health Transformation Program.

    Remote Measurements- If you can’t make it to measurements on site, you also have the option of completing measurements in the comfort of your own home.  All you need is access to a scale to enter a weight, and someone to take 2 photos of you.  The photos are kept private and confidential, and used ONLY to enter into our measurement software.

    To complete the measurements at home, you will also dress in a base layer of form fitting clothing with shoes removed.  You will click the link below, and you will enter some basic contact info, your height and weight, and you can take pictures directly (if you are using your phone), or retrieve a saved file. 

    Please note:  Height is entered in inches.  4 Feet is 48”, 5 Feet is 60” and 6 Feet is 72”.  i.e.- if you are 5 Feet 6 Inches tall, that is 66” entered in the height.  6 Feet 2 Inches is 74”.

    Notes about photos:

    The first picture is a forward pose with you facing the camera directly.  The second picture is a side pose with your RIGHT side facing the camera.

    Stand with feet a few inches apart, toes forward, arms relaxed at side, head facing forward.

    Picture should be taken from a straight, level angle (not from above or below- keep this in mind if you are propping your phone and using the timer).  There must be a little space above the head and below the feet so the entire body is in the picture, but not too much space which would mean the image is too far away.  

    Ideally a room with decent lighting (or flash), and the picture should not be too blurry.

  • Step 4 - Setting up Your Warrior Fitness App

    Once you fill out your Health History Questionnaire, Damien will use the responses to create your exercise program.  For a 12 Week Challenge, the programs are released over a few day period just prior to the start of the challenge.  For the Health Transformation program, you should receive your program within a week of filling out your questionnaire.  If you have not received an email with a link to download the app by then, contact us. (Always check your junk/spam email folder first).

    Once you receive the welcome email from Trainerize, it will contain a link to download the app, and instructions for how to setup your account.  If you are having trouble during any part of the setup process, check out the tutorial videos below.  If you still cannot access your account, contact us. 


  • Step 5 - Get ready to become a Warrior!

    Being a Warrior is not about how strong you are or how "In-Shape" you are. It's about your mindset. 

    It's about what you are willing to do to become the best version of YOU. 

    We will provide you with every resource and tool to help you. But it's up to you to take those tools and transform YOURSELF into the Warrior you're capable of being. 

    We expect you to push yourself, ask questions, take advice and challenge your old belief systems about yourself. 

    Do all that and you are a Warrior!
